Thank you again for taking the time to complete our volunteer forms. Completing the remaining four documents will help us to collect the information we need to reach you and to place you in a meaningful role that best suits your skills and Community Action Marin's needs.

I hereby acknowledge that participating as a volunteer with Community Action Marin is voluntary and done at one’s own free will. I understand and agree that Community Action Marin is not responsible for any injury, illness, harm, or property damage arising out of any volunteer activities, including actions caused by ordinary negligence, accidents, or intentional acts of others.

Further, I am aware that participation in volunteer activities involves certain risks, including, but are not limited to, serious injury and death and agree to accept all risks of participation. I hereby release, waive, and discharge Community Action Marin and its officers, directors, employees, agents, and volunteers of and from any and all claims which may arise in connection with such volunteer efforts or my participation therein.

I understand that this document is intended to be as broad and inclusive as permitted by the laws of the state of California. I release Community Action Marin, its contractors, and its employees from liability for any claims by me or any third party in connection with my participation and/or for the participation of the undersigned minor children.

I agree that, to my knowledge, I am in good health and suffer no impairments which may prevent my participation in volunteer activities.

Volunteers must complete the waiver prior to volunteer service. If the volunteer is under 18 years of age, a parent or legal guardian must sign and print the name(s) of the minor(s) volunteering.

All information and records containing medical. contact, and financial information about individuals, families, clients, vendors, and community members associated with Community Action Marin are to be kept completely confidential. Information given by clients to an Agency volunteer is confidential and in the absence of a written waiver, may only be disclosed to an agency employee or volunteer who needs such information to provide proper service to the client of for purpose of supervising or evaluating the provision of such services. Agency volunteers may also be privy to information that is considered confidential. This may include information about staff members, parents, children, proposed program plans, strategies for program implementation, and relationships with community members or other organizations. I understand that I share the responsibility of maintaining confidentiality of any child, parent, staff, or other volunteer information. I understand that it is my responsibility to assure the rights and confidentiality of information both written and verbal. I understand that if I breach confidentiality that it will result in my dismissal as a volunteer for Community Action Marin. I have read. acknowledged. and understand the terms of this confidentiality statement. If under the age of 18, signature of legal guardian is required

I consent to the reproduction and use of my image or voice, or those of my child (the parties) if specified below, upon the terms hereinafter stated. I hereby grant Community Action Marin (the agency) its affiliates, representatives and assign, the irrevocable and absolute right and permission to use, reuse, publish, republish, and broadcast photographic or video recordings or still pictures of or in which the parties may be included (with or without names). This includes photos in whole or in part, whether composite or distorted in character or form, without restriction for inclusion in art, videos, and/or audio recordings, advertising, trade, or any other purposes. I further consent to inclusion in any internal or external printed, social media, or website material in conjunction with any of the foregoing.

I hereby waive any right that I may have to inspect or approve the finished product or products or the advertising copy or printed matter that may be used in connection therewith or the use to which it may be applied.

I hereby release, discharge, and agree to hold harmless the agency, its legal representatives and assigns, those for whom the agency is acting and those acting with the agency’s authority and permission, from any and all claims that may arise out of or in connection with the use of any of such photographs, recordings, portraits, or other materials, including, without limitation. This includes any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite or distorted form, whether intentional or otherwise, that may occur or be produced in the taking of any of such photographs, portraits, or other materials or in any subsequent processing thereof, as well as any use, reuse, publication, republication, or broadcast thereof.
