This agreement is intended to ensure that an understanding is established between Community Action Marin management and volunteers regarding the Agency’s policies and expectations.

Community Action Marin

We, Community Action Marin [the “Agency”], agree to accept the services of the volunteer stated above beginning on date indicated . As such, our agency commits to the following: 1. To provide accurate information, training, and assistance. 2. To provide adequate supervision and timely feedback. 3. To respect the skills and individual needs of the volunteer. 4. To promote an environment that encourages personal growth and a valued experience.

I, the volunteer named above, agree to serve as a Community Action Marin volunteer and commit to the following: 1. To volunteer at Community Action Marin on an ongoing basis (this agreement can be terminated by one or both parties at any time, and for any reason). 2. To attend required training and clearly communicate with my Agency contact/supervisor through email, phone, and/or other communication channels. 4. To adhere to Community Action Marin’s policies and procedures, which include but are not limited to, embodying respect and cultural humility when working with community, abiding by Agency health and safety protocols, and maintaining confidentiality of client information. 5. To keep my Agency contact/supervisor informed of conflicting obligations which may prevent fulfillment of service or provide adequate notice in the event I am unable to follow through with a commitment so that alternate arrangements can be made.

Thank you again for taking the time to complete our volunteer forms. Completing the remaining four documents will help us to collect the information we need to reach you and to place you in a meaningful role that best suits your skills and Community Action Marin's needs.

Volunteer Release of Liability Waiver

I hereby acknowledge that participating as a volunteer with Community Action Marin is voluntary and done at one’s own free will. I understand and agree that Community Action Marin is not responsible for any injury, illness, harm, or property damage arising out of any volunteer activities, including actions caused by ordinary negligence, accidents, or intentional acts of others.

Further, I am aware that participation in volunteer activities involves certain risks, including, but are not limited to, serious injury and death and agree to accept all risks of participation. I hereby release, waive, and discharge Community Action Marin and its officers, directors, employees, agents, and volunteers of and from any and all claims which may arise in connection with such volunteer efforts or my participation therein.

I understand that this document is intended to be as broad and inclusive as permitted by the laws of the state of California. I release Community Action Marin, its contractors, and its employees from liability for any claims by me or any third party in connection with my participation and/or for the participation of the undersigned minor children.

I agree that, to my knowledge, I am in good health and suffer no impairments which may prevent my participation in volunteer activities.

Volunteers must complete the waiver prior to volunteer service. If the volunteer is under 18 years of age, a parent or legal guardian must sign and print the name(s) of the minor(s) volunteering.
