This form will take no more than 5 minutes to complete and there are only 5 questions that are required, but the more information you can provide, the better we can help you with services right away.

We will ask you about your estimated annual income (we do not require documentation); for households with multiple family members, please include family members’ names and dates of birth. To view the form in Spanish, please click the button at the top right corner of the form.

If you need help now or can’t attend one of our live orientations, we have recorded this orientation from a previous session. You can learn about our services and programs by watching the full video 

To obtain services, please submit the form below to self-certify that you have watched the orientation video. A client intake specialist will follow up with you once you submit your form. 

Option #2: Register for a live online Agency Orientation

Join us online to learn how our services can help you reach your goals. Participants will learn about the many different programs and services we offer and how we can be your partner on the path to thriving. We welcome questions from participants about how we can help them. Completing this orientation will help you know what we offer and help us know how to help you.

SparkPoint coaching is the only program that requires the completion of an agency orientation. Filling out the intake form in Step 1 will prompt a team member to contact you to support your access to all services.
